CUTANEOUS DIPHTHERIAย ย ICD-10: A30

โ€ข Etiology. Corynebacterium diphtheria. Cases in industrialized countries extremely rare.
โ€ข Pathogenesis. Localized infection caused by toxigenic and nontoxigenic strains. Acute infection may involve any mucous membrane or skin wound. Toxin causes myocarditis and peripheral neuropathy.
โ€ข Clinical Manifestations. Cutaneous: Nonspecific wound. Thick gray membrane in the pharynx. Myocarditis, arrhythmias. Polyneuritis involving cranial nerves: Diplopia, slurred speech, or difficulty swallowing.
โ€ข Diagnosis. Made by isolation of C. diphtheria on the culture of the wound.
โ€ข Treatment. Penicillin, erythromycin, or antitoxin. Vaccination. Immunity to vaccine wanes over time. Decennial boosters are recommended.
