皮膚白喉 (cutaneous diphtheria)，ICD-10：A30。
- 病原為白喉棒狀桿菌 (Corynebacterium diphtheria)。
- 在工業化國家中病例極為罕見。
流行病學與致病機轉
由產毒素 (toxigenic) 與不產毒素 (nontoxigenic) 菌株所引起的局部感染。急性感染可侵犯任何黏膜 (mucous membrane) 或皮膚傷口 (skin wound)。毒素 (toxin) 會引起心肌炎 (myocarditis) 與周邊神經病變 (peripheral neuropathy)。
臨床表現與分類
- 皮膚表現 (Cutaneous)：非特異性的傷口 (nonspecific wound)。
- 咽部出現厚實的灰色膜 (thick gray membrane in the pharynx)。
- 心肌炎 (myocarditis)、心律不整 (arrhythmias)。
- 侵犯腦神經 (cranial nerves) 的多發性神經炎 (polyneuritis)：複視 (diplopia)、言語含糊 (slurred speech) 或吞嚥困難 (difficulty swallowing)。
診斷與鑑別診斷
藉由傷口培養 (culture of the wound) 分離出白喉棒狀桿菌 (C. diphtheria) 來確立診斷。
治療與處置
- Penicillin、erythromycin 或抗毒素 (antitoxin)。
- 疫苗接種 (vaccination)。對疫苗的免疫力會隨時間減弱 (immunity to vaccine wanes over time)。
- 建議每十年追加一劑 (decennial boosters are recommended)。
